Inchicore Medical Centre is happy to accept new patients, including Medical Card/Doctor Visit Card holders and Private Patients.

Priority is offered to patients who do not have a GP in the Dublin area, and at present we are only accepting applications for those living in the local area.

If you hold a medical card, you should also complete a “HSE Change of Doctor” form which will be stamped by the surgery and sent to the HSE. This will result in a new medical card, in the name of one of our doctors being sent to you.

To register, simply complete our registration form.

If you have a GP elsewhere, please complete our Records Transfer form, which will allow us to seek your old medical records.

If you have a Medical Card/Doctor Visit Card, please complete the Change of Doctor form, and forward to us with your other documents.
